Subglacially precipitated carbonates record geochemical interactions and pollen preservation at the base of the Laurentide Ice Sheet on central Baffin Island, eastern Canadian Arctic

The mineralogy and isotopic compositions of subglacially precipitated carbonate crusts (SPCCs) provide information on conditions and processes beneath former glaciers and ice sheets. Here we describe SPCCs formed on gneissic bedrock at the bed of the Laurentide Ice Sheet (LIS) during the last glacial maximum on central Baffin Island. Geochemical data indicate that the Ca in the crusts was likely derived from the subglacial chemical weathering Ca-bearing minerals in the local bedrock. C and Sr isotopic analyses reveal that the C in the calcite was derived predominantly from older plant debris. The δ18O values of the SPCCs suggest that these crusts formed in isotopic equilibrium with basal ice LIS preserved in the Barnes Ice Cap (BIC). Columnar crystal fabric and the predominance of sparite over micrite in the SPCCs are indicative of carbonate precipitation under open-system conditions. However, the mean δ18O value of the calcite crusts is ~ 10‰ higher than those of primary LIS ice preserved in the BIC, demonstrating that SPCCs record the isotopic composition of only basal ice. Palynomorph assemblages preserved within the calcite and basal BIC ice include species last endemic to the Arctic in the early Tertiary. The source of these palynomorphs remains enigmatic.

The rate of autism spectrum disorders (ASDs) and brain abnormalities was analyzed in 31 individuals (15 males, 16 females; age range 1mo to 31y, mean age 8y 11mo) with CHARGE association, as part of a multidisciplinary study. A meticulous neuropsychiatric examination was performed, including standardized autism diagnostic interviews. Judgement regarding ASDs was impossible in three infants and three patients who were deaf and blind. Five individuals met diagnostic criteria for autism, five for an autistic-like condition, and seven for autistic traits. Brain abnormalities were indicated in almost three-quarters of examined individuals, and midline abnormalities of the forebrain in one-third. Awareness of the coexistence of CHARGE and ASDs is important in habilitation care in CHARGE. Moreover, the results indicate that a subgroup of ASDs may be associated with errors in early embryonic brain development.

The prevalence of autistic disorder was analysed in 25 individuals with Möbius sequence, a disorder with brain-stem dysfunction. The sample consisted of 18 males and seven females (20 participants were aged 2 to 22 years, and five were aged 1, 19 and 23 months, and 55 years old). Participants were recruited after a nationwide call and were part of a multidisciplinary study of individuals with Möbius sequence. They were given a meticulous neuropsychiatric examination including standardized autism diagnostic interviews. Ten individuals had an autistic spectrum disorder. Six of these met all diagnostic criteria for autism. In 23 individuals cognitive development could be assessed. Eight of those 23 patients had clear learning disability and six individuals were functioning in the normal but subaverage range. Autistic spectrum disorder and learning disability occurred in more than a third of the examined patients. Considering the hospital-based nature of the sample, these findings may be overestimates. Nevertheless, awareness of this coexistence is important in the diagnosis and habilitation care of children with Möbius sequence. Moreover, the results provide further support for the notion of a subgroup of autistic spectrum disorders being caused by first trimester brain-stem damage.

To assess baseline health status of a medical school employee population and to assess this population's acceptance of vaccination and other interventions to reduce risk of disease transmission.
Design:A retrospective review of an employee health records database for a 4-year period.
Setting:A large, urban university hospital.
Participants:5,007 employees screened by employee health for immunity to vaccine-preventable illnesses and tuberculosis.
Results:9.4% of the employees had positive tuberculin skin tests, with a conversion rate of 6.4% for those who had negative tests within the previous 2 years. Two individuals were identified who had active pulmonary tuberculosis. Fewer than 10% of the individuals for whom isoniazid chemoprophylaxis was recommended completed the 6 months of therapy. Most clinical employees (96.1%) did not have a history of prior hepatitis B virus (HBV) infection or immunization, but 77% of them subsequently completed the vaccination series. Most employees with a negative history for infection with or immunization against rubella, rubeola, and varicella had serological evidence of immunity (90.2%, 97.9%, and 87.2%, respectively).
Conclusions:Review of aggregate employee health databases may assist individuals who must establish strategies for prevention of occupational illness and disease transmission in this specialized setting. While many employees at risk for HBV complete the vaccination series, strategies for improving this rate could be helpful. Substantial work is needed to analyze reasons why so few individuals for whom isoniazid chemoprophylaxis is recommended complete the therapy, and strategies tailored to the impediments identified should be implemented.
Risk of Infection Due to Central Venous Catheters: Effect of Site of Placement and Catheter Type

OBJECTIVE:
To determine the influence of catheter site and type (single- vs triple-lumen) on infection rates associated with central venous catheterization.
DESIGN:Prospective observational study of all nontunneled central venous catheters over a 28-month period. Data collected included patient characteristics, insertion site, catheter type, and receipt of parenteral nutrition. End points were clinical infection (bacteremia or site infection) and catheter contamination (clinical infection or colonization with >15 colonies on semiquantitative culture).
SETTING:Medical-surgical wards of Veterans' Affairs hospital.
RESULTS:Three hundred catheters were inserted into 204 patients. Seventy percent were inserted into upper-body sites, and 30% were inserted into the femoral vein. Forty-five percent were triple-lumen catheters. Bacteremia occurred in 2.7% of catheter insertions; insertion-site infections developed in 1.3%, and catheter colonization developed in 12%. Catheter contamination was associated with emergent insertion (odds ratio [OR], 6.2; 95% confidence interval [CI95], 1.1-36.7; P=.04) by logistic regression and with femoral location (hazard, 4.2; CI95, 2.0-8.8; P=.0001) and history of transplantation (hazard, 2.8; CI95, 1.1-6.7; P=.024) by Cox regression. Clinical infection was not associated with any of the risk factors evaluated, although there was a trend for association with femoral location by Cox regression (hazard, 4.7; CI95, 0.82-26; P=.08). We did not identify an association between infection and use of triple-lumen catheters or parenteral nutrition.
CONCLUSION:Our data support an association between intravenous catheter contamination and insertion at a femoral site.

A protocol for the generation of high yields of viable protoplasts has been developed for several highly branched (colonial) strains of the Quorn® myco-protein fungus, Fusarium graminearum A3/5. Driselase was found to produce higher protoplast yields (ca 109 g−1 wet weight) than the other lytic enzymes tested (Glucanex, Novozyme, β-glucuronidase, Sigma lytic enzyme, or ICN lytic enzyme), although yields differed for the various strains. Protoplast regeneration frequencies of 25–50% were observed when glucose (1·0 M) or sucrose (1·0 M) was used as the osmotic stabilizer. A highly branched strain of F. graminearum CC1-5, which grows better in submerged culture than the more sparsely branched wild-type strain (A3/5) was transformed using the hygromycin B resistance plasmid pAN7-1.
Isotopic and molecular characterization of modern and fossil proteins in ostrich eggshell

Stable carbon and nitrogen isotopes in fossil ostrich eggshell (OES) proteins have the potential to provide paleodietary information on ostriches through the Late Pleistocene in arid and semiarid regions of the Old World. Carbon isotopes may be used as a monitor for floral composition (C3 vs. C4 plant abundance), for example. The stable isotope composition of the organic matter in modern OES and Middle Holocene fossil OES samples from Bir Tarfawi (an archaeological site in southern Egypt) are presented, as well as the nature of modern proteins in OES and the degree to which they are preserved in the fossil samples.
The organic fraction was extracted from OES samples and subsequently desalted by (1) decalcification in 0.5 N EDTA, followed by dialysis against water in >6-8,000 molecular weight cut off (MWCO) tubing, or (2) hydrolysis in 6N HCl for 20 minutes at 153°, followed by the addition of concentrated HF to facilitate the precipitation of CaF2. There is good agreement between the stable isotope results of the two organic fractions from modern OES samples (δ13C = −23.51±±0.76 and −23.53±±0.39, and δ15N = +6.23±±0.15 and +6.42±±0.33 for the >6-8,000 MWCO and HF preparations, respectively). A 2± and 4± enrichment between ostrich diets and the organic fraction was found for carbon and nitrogen, respectively. These relationships support a vegetarian diet comprised almost exclusively of C3 plants. Result from the HF preparation show higher variability in both the δ13C and δ15N values in the fossil samples than in the modern. This increased variability may be due to isotopic fractionation accompanying protein degradation or to inadequate methodologies for isolating the bulk organic matter. These effects could be minimized by analyzing individual molecules throughout the study.
The >6-8,000 MWCO fraction of the modern and fossil OES samples were electrophoresed on SDS polyacrylamide gels. The modern sample shows predominant bands corresponding to apparent molecular weights of 14 kilodaltons (kD), 28 kD, and 35 kD, and a light smear at the top of the gel. Preliminary results of the fossil sample show a pronounced band corresponding to an apparent molecular weight of 80 kD, and a light smear over the whole gel. Results of an enzyme linked immunosorbent assay (ELISA) on the modern material demonstrate a positive antibody-antigen reaction between the >6-8,000 MWCO fraction and osteocalcin, osteonectin, and albumin.
Due to the “closed-system” nature of the ostrich eggshell matrix and the ease with which it can be accurately dated through the Late Pleistocene, it provides an ideal medium for studying protein diagenesis. Subsequent isotopic analyses, using the newly developed technologies of GC/IRMS, offer an alternative method for using the paleodiets of ostriches as a tool for paleoenvironmental reconstructions in Africa and Asia.
Household Archaeology at Cerén, El Salvador

In the summer of 1989, major discoveries were made at the site of Joya de Cerén, El Salvador, where sudden depositions of volcanic ash in a.d. 600 resulted in unusually favorable conditions of preservation. The theoretical framework for the research is household archaeology, the study of prehistoric household groups. Household archaeology, as applied to Cerén can take advantage of the extraordinary preservation to study households in terms of their key activities of (a) production, including food, implements, vessels, and structures; (b) “pooling,” including storage, distribution, maintenance, and curation activities; (c) transmission of knowledge and material goods including access to resources; (d) reproduction in both the biological and sociocultural senses; and (e) co-residence/membership in the functioning residential group. One of the major finds was a possible codex or Precolumbian manuscript.
Discordant Monozygotic Twins with Trisomy 13

Monozygotic twins with typical trisomy 13 are reported. Despite an identical karyotype, the twins were dimorphic for the presence of an omphalocele. Reasons for the rarity of MZ twins with trisomy 13 are presented. It is suggested that the presence of a chromosomal abnormality in MZ twins may predispose to dimorphism.
